- Slipping and falling while not on rope.
- Attaching rappel device to the wrong strand of rope.
- Old webbing at anchor rigging breaking.
- Rockslide.
- Hypothermia/drowning in potholes.
- Flash floods.
- Rappelling off the end of the rope.
- Losing control while on rappel.
- Improper rigging (failed block, etc.)
